Office Hours: 8:00 a.m. to 4:30 p.m., Monday through Friday.

Appointments: On your first visit, please arrive 30 minutes prior to your appointment time so you can complete all necessary paperwork. You should bring a list of current medications you are taking, or the actual medications, as well as copies of any relevant x-ray, ultrasound, mammogram or CT scan reports, including both the film and written report. This will assist the physicians in assessing your diagnosis.

Refills: Generally, we will give you the appropriate number of refills to last until your follow-up appointment. Please be sure to schedule your follow-up appointment before your prescription runs out.

Insurance: We participate in most health plans. Before scheduling your first appointment, please refer to your health plan booklet or contact your health plan directly to determine whether we are a participating provider.

If you have an HMO or POS health plan, it's likely that you will need an authorization form from your primary care physician prior to your appointment. Please check with your health plan to make sure all necessary arrangements are in place.

Billing: Please be prepared to pay the co-payment set by your health plan on the day of your appointment. After all insurance payments and deductions are made, you will receive a bill for the remaining amount. Please let us know if you have any concerns about your bill or if you'd like to make arrangements for payment plans.
